Range day with my rebuild = Epic fail

I had it reassembled only replacing the Cylinder Alignment Ball Spring, and the hand Spring from the Spring Kit.   During disassembly, the Crane would not slide out of the frame. SA/DA was fine after re-assembly.   Gauged right.   It appeared I did it!

All well until first shot. Jammed. Cylinder not rotating or ejecting. I used the 0.002" gauge to disengage the Bolt from the Cylinder and I was able to wiggle the cylinder out of the frame. I had to tap out casing out with wooden dowel. New cartridge to replace one fire. Now cylinder crane will not lock back into frame.

I have not yet dove into a detailed inspection.  

Any Ideas where to start?
