The 4th round of a "double tap" drill Kinda-sotrta FTF twice!

Any thoughts?

The drill was  3 sets of 2 rounds fired, as a double tap, double action, with a pause between each "set of two" rounds fired.

all rounds seemed to fire BUT on examining the fired cases the 4Th round (I have my "#1" chamber indexed) had zero primer marks!

BUT rounds 1~2 & 3~4 created an impact on the berm!

So, here's the question:

Has anyone ever found a D-W 15-2 to somehow "skip a chamber" when firing double action, very rapidly, but only between the first & second pair of a double-tap?

example:

6 chambers

1 & 2 fire just fine

Break

3 & 5 fire just fine,

6 fires leaving 4 unfired

BUT both "double taps" have produced an obvious (25 Yd) berm impact?

Trying to follow this interesting scenario, but I’m not exactly sure what is happening. I think the only way a chamber could be “skipped” in rapid double action fire would be either a weak bolt spring, or an excessively worn bolt. It would make sense that either condition may not show up in slow fire since the spring would have more time to engage. That’s the first thing I would check. Another possibility would be excessive crud buildup in the bolt mechanism thus not allowing proper engagement.

Thanks, I hadn't thought of that!

I did find the ejector rod had loosened about 1 1/2 turns though, but I can't figure out how that would play into the problem.

I don’t think the ejector rod backing out a little would have an effect on timing. I’ve made it a habit to check it every time I load the cylinder. A dab of nail polish on the threads seems to help for a while.
